Participant Waiver, Release & FalconEye Video Consent
1. Athlete and parent or guardian
2. Assumption of risk
Lacrosse is a physical, fast-moving contact sport. Training includes running, change of direction, stick and body contact, ground-ball play, and faceoff or draw work, using hard balls and sticks that move at high speed. I understand that participation carries inherent risks that cannot be fully eliminated, including but not limited to sprains, strains, fractures, cuts, bruises, dental and eye injuries, concussion and other head injuries, heat-related illness and, in rare cases, catastrophic injury or death.
I confirm the athlete is physically able to participate, will wear the protective equipment required for the activity, and will follow the trainer's instructions. I will tell the trainer about any injury, illness or condition that could affect the athlete's safe participation. I knowingly and voluntarily accept these risks on the athlete's behalf and my own. Initial:
3. Release of liability
In consideration of the athlete being allowed to participate, I, for myself, the athlete, and our heirs and representatives, release, waive and agree not to sue the trainer named above, and the trainer's employees, assistants, volunteers and the owners of any facility used for the session and FalconEye-Faceoff Intelligence System LLC and its owners and employees (together, the "Released Parties"), from any and all claims for injury, illness, death or property damage arising from the athlete's participation, including claims arising from the ordinary negligence of the Released Parties, to the fullest extent permitted by the law of the state named above. This release does not apply to gross negligence or intentional misconduct. If any part of this waiver is found unenforceable, the rest remains in effect. Initial:
4. Medical treatment
If the athlete is injured or becomes ill and I cannot be reached, I authorize the trainer to obtain first aid and emergency medical care for the athlete, including transport to a medical facility, and I agree to be responsible for the cost of that care.
FalconEye has no medical responsibility. FalconEye is a software provider only. It does not provide medical care, medical advice, injury assessment, clearance to play, or supervision of the athlete, and it is not responsible for the athlete's health, safety, injuries or medical treatment. Nothing produced by FalconEye should be relied on for any medical or health decision; consult a qualified medical professional.
5. FalconEye video analysis: acknowledgment and consent
The trainer uses FalconEye, a lacrosse film-analysis platform provided by FalconEye-Faceoff Intelligence System LLC ("FalconEye"), to record and review sessions. I understand and agree that:
- Filming. Sessions may be recorded on video. Recordings may show the athlete alongside other athletes in the same session.
- What is uploaded. The trainer may upload that video to FalconEye along with the athlete's name, position, jersey number and performance notes. The medical and emergency details on this form are not uploaded; this sheet stays with the trainer.
- AI analysis. FalconEye uses artificial intelligence, including models from Google and Anthropic, to break down technique and produce feedback. Those providers may not use the athlete's video to train their own models. AI feedback can be wrong, is reviewed by the trainer, and is not medical advice or a medical evaluation.
- Who sees it. The athlete's film and feedback are visible to the trainer and, where the athlete is part of a program, to that program's coaching staff. FalconEye does not sell personal information or use it for advertising.
- Control and deletion. The trainer controls this data. I may ask the trainer to delete the athlete's video and information at any time, or contact FalconEye directly at chris@falconeyefaceoff.com. FalconEye's full Privacy Policy is at falconeyefaceoff.com/privacy.
- FalconEye's role. FalconEye provides software only. It does not organize, run, supervise or staff training sessions and is not responsible for how sessions are conducted, or for any injury, illness or medical matter arising from them.
- Age limit. FalconEye is not used for any athlete under 13.
